Together, we will deliver a connected care experience​

• Simple VirtualCheckup® and a video consultation with a licensed nurse practitioner (NP)​
• Lab testing​
• Wellness check​
• Health risk assessment​
• Personal health action plan and education​
• If members are assessed with conditions or risk factors, NPs can immediately refer and enroll them in clinically appropriate programs​
• Referrals into optimal health improvement programs​
  Catapult nurse practitioners direct participants into clinically appropriate employer and plan-sponsored programs​
• Connection back to participant’s PCP​
  Results sent securely to each participant’s PCP; those without a PCP are assisted in finding one​
