Service Overview

What is the Cardiometabolic Health Program?

The Cardiometabolic Health Program is our most complete offering to address your full member population, including multi-condition support and weight management, delivered with the highest clinical quality and delivered by a dedicated care team combined with proven technology and behavior change tools – all in a single program and member experience.

Who is eligible for the Cardiometabolic Health Program?

Teladoc Health partners with clients to identify eligible members for any one or combination of the following conditions. Eligibility requirements vary for each solution:

• Diabetes— requires diagnosis of type 1 or type 2 diabetes

• Hypertension—requires diagnosis of hypertension

• Prediabetes— must meet CDC diabetes prevention program (DPP) qualification criteria

• As stated on the CDC website, an individual must meet the following requirements:

• Be at least 18 years old -AND-

• Be overweight (BMI≥25; ≥23 if Asian) -AND-

• Have no previous diagnosis of type 1 or type 2 diabetes -AND-

• Have a blood test result in the prediabetes range within the past year:

• Hemoglobin A1c: 5.7%-6.4% -OR-

• Fasting plasma glucose: 100-125 mg/dL -OR-

• Two-hour plasma glucose (after a 75gm glucose load): 140-199 mg/dL -OR-

• Be previously diagnosed with gestational diabetes

• Our program enrollment process will use the CDC-approved screening questionnaire

• Weight Management – must meet BMI requirements, which can vary by client

Is provider oversight included in the Cardiometabolic Health Program?

Yes, provider-based care for high-risk members is included. Clients are allowed to opt-out if

preferred.

Is weight management included in the Cardiometabolic Health Program?

Yes, weight management is included in the program, and you have the option to include our

obesity care offerings for GLP-1 and lifestyle support as an option at an additional cost.
